About the role

As a Licensed Practical Nurse (LPN) at 3HC, you will administer skilled nursing care to home health and hospice patients, following individualized plans of care delegated by the RN. You will provide meaningful one-on-one care, develop relationships with patients and team members, and contribute to a positive work culture that supports both personal and professional growth.

Responsibilities

  • Adhere to and support 3HC's Compliance Program and Code of Ethics, ensuring all actions align with applicable laws and regulations.
  • Create positive experiences for internal and external customers, displaying courtesy, tact, and knowledge of agency services.
  • Assist in achieving positive patient outcomes and providing comfort to patients and families by:
    • Performing all skills, treatments, and procedures as outlined in the patient's plan of care.
    • Observing and reporting signs, symptoms, and reactions to treatments, including changes in physical or emotional condition.
    • Reinforcing patient and family teaching completed by the RN regarding nursing care needs and procedures.
    • Assisting the RN or physician with specialized procedures and duties.
    • Supporting the emotional needs of patients and their families, ensuring continuity of care.
    • Reporting outcomes of unscheduled visits to the RN within 24 hours.
  • Conduct ongoing observation of the patient's physical and mental health and home environment by:
    • Gathering and documenting data according to the patient's plan of care.
    • Recognizing relationships between gathered data and the patient's health status.
    • Determining the need for immediate nursing intervention based on data and plan of care.
  • Document patient care electronically in the medical record, including:
    • Services rendered and clinical progress notes.
    • Conferences with physicians and other disciplines.
    • Changes in the patient's condition or home situation.
    • Revisions to the care plan, supervised by the RN.
    • Daily submission of all paperwork, including time and travel logs.
    • Updating the medication profile with each medicinal change.
    • Staying current on reimbursement criteria, hospice criteria, and charting requirements.
    • Responding to documentation correction requests within 48 hours.
  • Collaborate with supervisors to meet agency clinical needs, including rotating weekend, after-hours, and holiday on-call duties with RN backup.
  • Maintain regular communication with supervisors and follow the chain of command for patient-related issues.
  • Stay current on patient care practices and improve nursing skills by:
    • Participating in staff meetings, patient care conferences, record audits, and quality improvement activities.
    • Attending interdisciplinary team conferences at least 75% of the time.
    • Completing mandatory in-services and continuing education requirements.
    • Engaging in continuing education, professional organizations, and individual research.
  • Demonstrate personal and professional growth by:
    • Attending workshops, seminars, and webcasts to stay updated on job-related rules and regulations.
    • Conducting individual reading and research.
    • Recommending educational opportunities for staff.
  • Use agency resources cost-effectively, monitor waste, and ensure the proper and safe use of supplies and equipment.
  • Adhere to 3HC's Personnel Policy and perform other duties as assigned to support agency operations and growth.

Requirements

  • Current Licensed Practical Nurse (LPN) license to practice nursing in North Carolina.
  • Minimum of two years of nursing experience in an acute care setting preferred.
  • Current CPR certification (BCLS).
  • Dependable, with the ability to interact pleasantly and tactfully with patients, families, staff, providers, and the public.
  • Competence in performing delegated tasks to facilitate positive patient outcomes as outlined in the patient's plan of care.
  • Valid driver's license and reliable transportation.
