About the Role
The licensed practical nurse (LPN) provides nursing care to assigned patients under the direct supervision of the registered nurse (RN) or other assigned supervisor and accepts delegation from the RN/assigned supervisor in meeting the needs of the patient/family. The LPN collects data through observation and communicates information to assist the RN and/or provider in patient assessment and care planning. The LPN participates as a member of the health care team and delivers care in accordance with the patient care plan, policies and procedures of the organization, and principles of relationship-based care.
The LPN possesses excellent communication skills; is skillful in mentoring and instructing; and may participate on committees or projects, including quality improvement projects.
Responsibilities
- Provide nursing care to assigned patients under RN supervision.
- Collect and communicate patient data to assist in assessment and care planning.
- Participate as a member of the healthcare team, accepting delegation from RNs and providers.
- Deliver care in accordance with the patient care plan and organizational policies.
- Mentor and instruct team members as needed.
- Participate in quality improvement projects and committees.
Requirements
- Excellent verbal and written communication skills.
- Ability to work in a team environment.
- Flexibility to work days, evenings, nights, holidays, weekends, and on-call shifts.
- Ability to adapt to unpredictable situations in the work setting.
- Basic computer skills.
Preferred Qualifications
- Prior experience with electronic medical record (EMR) systems.
- One (1) year of LPN experience within the last five years.
- Long-term care experience.
Schedule
13-week assignment with shifts from 6AM – 6PM or 6PM – 6AM. Every 3rd weekend is required.
