LPN - Adults with Developmental Disabilities Program

Monday - Friday, 10 a.m. to 2 p.m.

About the role

Provide direct health-related care and health monitoring/management, including administering medications, performing treatments, and monitoring vital signs. Coordinate with families, other healthcare professionals, and train direct care staff.

Responsibilities

• Practices job requirements accurately, timely, and regularly
• Observes and maintains appropriate professional boundaries with members, co-workers, and supervisors
• Attends all scheduled meetings as assigned
• Meets relevant continuing educational/training requirements
• Completes all required documentation/paperwork accurately and within established time frames
• Completes all assigned College of Direct Support and Relias trainings
• Participates in Medication Management, including administration of medication, maintenance of MAR's, timely requests of medication refills, integrity and orderliness of the medication cart/lockbox, and appropriate disposition of outdated medications as per policy
• Participates in members' intake process ensuring NJFH receives all necessary medical documentation prior to the start of program
• Reviews medical documentation for each member a minimum of twice per year to ensure it is up-to-date and complete
• Maintains documentation, including medical records, appointment schedules, and nurses' notes
• Provides feedback and shares concerns regarding supported individuals with the site manager, Day Habilitation Director, or other administration
• Ensures the program receives regular medical updates regarding each individual at least once per year or more depending on health status
• Participates in the formation of a Nursing Care Plan (NCP) for each supported individual if necessary and provides needed staff training
• Provides individual care as directed based on each individual's ISP or NCP
• Participates in health training for members, including recognizing signs and symptoms of medication side effects, disease process and management, infection control & prevention (e.g., handwashing)
• Monitors vital signs and response to medication and treatments as necessary
• Provides guidance, feedback, and training to direct care staff based on needs of supported individuals
• In collaboration with members' family and staff, monitors chronic diseases and reports signs and symptoms of complications
• Participates in supportive care, including chronic pain management, hospice, and palliative care
• Ensures all individuals' medical records comply with applicable state regulations and requirements of NJFH
• Supports the interdisciplinary approach to members' care and participates in team processes as directed by program administration
• Provides First Aid/CPR and AED training to agency staff and participates in new employee orientation and training
• Provides annual education to members and staff, including medication safety, side effects, winter cold safety, summer heat & sun safety, and heat intolerance
• Travels to nearby program sites as required

Requirements

• Completion of an accredited Practical Nursing/LPN program
• NJ State Licensure
• First Aid/CPR/AED Instructor certification or ability to become instructor within 60 days of employment
• Valid driver's license with a clean record
