About the role

Vinfen is a nonprofit organization providing community-based services to individuals with various conditions. The role involves providing nursing care, administering medication, and assisting individuals in becoming more integrated into their communities.

Responsibilities

  • Nursing Care:
    • Develops and implements person-centered treatment care plans.
    • Assists with activities of daily living.
    • Makes individualized assessments and provides skilled nursing interventions.
    • Administers medication and treatments.
    • Provides transportation and accompanies individuals to appointments.
    • Maintains nutritional needs and ensures timely ordering of supplies and equipment.
    • Utilizes behavioral strategies and follows clinical plans.
  • Supervision:
    • Supervises Certified Nursing Assistants (CNAs).
    • Maintains accuracy and compliance with ISP goals.
    • Provides relevant nursing education and training.
  • Communication:
    • Maintains communication with primary care providers, families, and other significant relationships.
    • Communicates effectively with other Vinfen employees and external agencies.
    • Utilizes emergency and on-call systems.
    • Reports any changes in status to appropriate individuals.
  • Documentation:
    • Documents nursing care and treatments.
    • Documents medication administration.
    • Ensures completeness and accuracy of documentation.
    • Reports incidents, including medication errors.
  • Environment:
    • Maintains a safe, clean, and welcoming environment.
    • Maintains compliance with regulatory requirements.
    • Respects clients' preferences and choices.
    • Maintains medical equipment and supplies.
  • Community Integration:
    • Involves individuals in community outings and events.
    • Respects individual preferences.
  • Advocacy/Relationships:
    • Advocates for individuals served.
    • Liaises with other Vinfen departments and external agencies.
    • Participates in treatment planning and ISP development.
  • Professional Development:
    • Acts as a positive role model.
    • Attends trainings and conferences.

Qualifications

  • Bachelor's degree in Social Work, Psychology, or related field preferred, but experience may substitute.
  • High school diploma or GED required.
  • L.P.N. license required.
  • One to two years supervised practical experience, including internships, in a clinical or rehabilitation setting preferred.
  • Experience working with people with psychiatric conditions and/or co-occurring disorders strongly preferred.
  • Strong time management and writing skills.
  • At least 21 years old and a current, valid US drivers' license for at least 6 months.
  • CPR and First Aid certification within 2 weeks of employment.
  • MAP certification within 150 days of employment.
  • Passes CORI, reference, multi-state background, and driving record checks.
