Job Objective
Coordinates delivery of nursing and social services for adult day participants
Minimum Qualifications
- Education: Graduate of accredited nursing school
- Experience: 2-3 years of nursing experience. Experience working with the elderly and those with dementia/Alzheimer's Disease preferred; work in a community-based setting and knowledge of services and entitlements for frail elderly are a plus.
- Licensure: State of Connecticut Nursing License; CPR certification

- Availability of personal vehicle for work-related travel (mileage reimbursed)
- Travels primarily within Fairfield and New Haven counties
- All other related duties as assigned
Essential Job Functions
- Conduct new client intakes and assessments to determine the appropriateness of referrals and encourage participation at start of program and once enrolled
- Conduct/assess the nutritional, medical, physical and mental functional needs of the clients during the initial assessment and ongoing as necessary within scope of nursing practice
