LPN - Adult Day Program Nurse

About the role

Coordinates delivery of nursing and social services for adult day participants.

Responsibilities

• Conduct new client intakes and assessments to determine the appropriateness of referrals and encourage participation at start of program and once enrolled.
• Conduct/assess the nutritional, medical, physical and mental functional needs of the clients during the initial assessment and ongoing as necessary within scope of nursing practice.
• All other related duties as assigned.

Qualifications

• Graduate of accredited nursing school.
• State of Connecticut Nursing License.
• CPR certification.

Experience

• 2-3 years of nursing experience.
• Experience working with the elderly and those with dementia/Alzheimer's Disease preferred.
• Work in a community-based setting and knowledge of services and entitlements for frail elderly are a plus.

• Ability to travel to client homes and other community sites as necessary related to networking and intakes.
• Availability of personal vehicle for work related travel (mileage reimbursed).
• Travels primarily within Fairfield and New Haven counties.

Schedule

Part Time: 22.5 hours / 3 days per week.
