About the role
Coordinates delivery of nursing and social services for adult day participants.
Responsibilities
- Conduct new client intakes and assessments to determine the appropriateness of referrals and encourage participation at start of program and once enrolled.
- Conduct/assess the nutritional, medical, physical and mental functional needs of the clients during the initial assessment and ongoing as necessary within scope of nursing practice.
- All other related duties as assigned.
Qualifications
- Graduate of accredited nursing school.
- State of Connecticut Nursing License.
- CPR certification.
Experience
- 2-3 years of nursing experience.
- Experience working with the elderly and those with dementia/Alzheimer's Disease preferred.
- Work in a community-based setting and knowledge of services and entitlements for frail elderly are a plus.

- Ability to travel to client homes and other community sites as necessary related to networking and intakes.
- Availability of personal vehicle for work related travel (mileage reimbursed).
- Travels primarily within Fairfield and New Haven counties.
Schedule
Part Time: 22.5 hours / 3 days per week.
