LPN - Adult Crisis Therapeutic Home

About the role

We’re seeking a compassionate and dedicated LPN to join our Virginia REACH team designed to provide support to individuals in our Adult Crisis Therapeutic Home (ACTH) located in Woodbridge, VA.

Responsibilities

• Administer medications
• Conduct nursing assessments
• Provide health education
• Ensure compliance with regulatory standards
• Collaborate with interdisciplinary teams
• Maintain accurate documentation
• Support individuals’ overall health and wellness

Requirements

• Current valid, unrestricted Practical Nurse license in the state of Virginia
• Valid driver’s license and car insurance
• Ability to provide care in a crisis group home setting
• Experience with MS Office suite and data entry
• Strong verbal and written communication skills
• Passion for helping others

Qualifications (Preferred)

• Knowledge of the mental health (MH) and intellectual/developmental disabilities (I/DD) systems of care
• Experience with crisis prevention and intervention services

Pay

Starting at $27.00 per hour (based on experience)

Schedule

Monday – Friday, 11:00 pm – 7:00 am
