About the role

At Easterseals PORT Health (ESPH), we are seeking compassionate and motivated full-time Licensed Practical Nurses to join our Virginia REACH team supporting individuals with intellectual/developmental disabilities and co-occurring behavioral health needs at our warm, home‑like Crisis Therapeutic Home (CTH) in Staunton, VA. This role offers a $10,000 sign‑on and retention bonus for new hire LPNs.

Responsibilities

• Provide compassionate medical and nursing support to individuals with intellectual/developmental disabilities and co‑occurring behavioral health needs
• Administer medications safely and accurately in accordance with clinical protocols
• Conduct nursing assessments
• Educate and empower individuals regarding their mental and physical health
• Collaborate closely with interdisciplinary team members to ensure coordinated, person‑centered care
• Maintain thorough and timely documentation of all nursing services and observations
• Uphold compliance with all regulatory and clinical care standards to ensure the highest quality of service

Requirements

• Current valid, unrestricted Practical Nurse license in the state of Virginia
• Knowledge of mental health and I/DD systems of care preferred
• Experience with crisis prevention and intervention services preferred
• Valid driver’s license and car insurance for occasional transportation to appointments

• Strong communication and documentation abilities

Schedule

• Monday – Friday, 3pm‑11pm
• Monday – Friday, 11pm‑7am

Pay

Competitive rate: $26.00 – $31.00 per hour for this full‑time, non‑exempt position. Sign‑on and retention bonus: $10,000 paid out upon successful completion of 90 days, 6 months, and 1 year.
