About the role
Seeking a Licensed Practical Nurse (LPN) to provide safe, patient-centered care in home settings for adult patients with G-tube, Trach, and Ventilator needs. The role involves utilizing the nursing process to assess, plan, implement, and evaluate care, including collecting and analyzing patient data, and providing skilled nursing services according to prescribed orders.
Responsibilities
- Assess signs and symptoms to identify physiological and psychosocial changes.
- Document and interpret nursing diagnoses and patient information.
- Administer care in compliance with healthcare plans and professional standards.
Requirements
- Current LPN/LVN license in the practicing state.
- Valid BLS certification.
- Current TB or Chest X-Ray.
Schedule
Work in Suffolk or Portsmouth, primarily daytime shifts.
