LPN Acute Care Transition Nurse - Night Shift

Role Overview: This position is for a Licensed Practical Nurse (LPN) or Vocational Nurse (LVN) providing direct patient care in an acute care setting, primarily on specialty Med-Surg units with moderate to high patient acuity. Responsibilities: Deliver top-of-scope nursing care using the nursing process, collaborate within a multidisciplinary team to achieve optimal outcomes, prioritize patient wellness, model proficiency through precepting, and contribute to safety and quality at the point of care. Qualifications & Skills: Graduate of an accredited Practical or Vocational Nursing program, current state licensure, and BLS certification through the American Heart Association or Red Cross. Ability to work night shifts and adapt to a fast-paced, innovative clinical environment. Key Duties: Provide evidence-based care in acute care units, participate in ongoing education, and support team-based approaches to patient care.
