Job Summary
As a Licensed Practical Nurse (LPN) at Lexington Premier Nursing and Rehab, you will be responsible for assessing patient needs, providing direct nursing care, and collaborating with the healthcare team to ensure the best possible outcomes for our residents.
Responsibilities
- Patient Care: Provide direct nursing care to residents, including administering medications, wound care, and treatments as prescribed by the physician.
- Monitoring and Assessment: Monitor residents' vital signs, document their progress, and report any significant changes to the Charge Nurse or Director of Nursing.
- Care Planning: Assist in the development and implementation of individualized care plans, ensuring they are tailored to meet each resident's needs.
- Communication: Maintain clear and accurate documentation of patient care, communicate effectively with residents, families, and the healthcare team.
- Compliance: Adhere to all facility policies, procedures, and regulatory requirements, including infection control and safety protocols.
- Support: Provide emotional support and education to residents and their families, helping them understand care plans and health conditions.
- Team Collaboration: Work collaboratively with other healthcare professionals, including RNs, CNAs, physicians, and therapists, to deliver coordinated and comprehensive care.
Qualifications
- Licensure: Valid LPN license in Kentucky
- Education: Graduation from an accredited LPN program
- Experience: Previous experience in a skilled nursing facility or similar healthcare setting preferred
- Certifications: Current CPR/BLS certification required. Additional certifications may be required based on state regulations
- Communications: Excellent verbal and written communication skills. Ability to work collaboratively with a diverse team of healthcare professionals and build positive relationships with residents and their families
- Physical Requirements: Ability to lift and move patients, perform physical assessments, and stand for extended periods
- Skills: Ability to perform assessments, Administer medications, Provide wound care, Excellent communication and interpersonal skills are essential
