About the Role

We are seeking a Licensed Practical Nurse (LPN) to join our Adult Community Clinical Supports (ACCS) team and help advance our mission of empowering people to live healthier, more fulfilling lives.

Responsibilities

  • Participate in data collection related to the status and needs of individuals served
  • Provide direct care within scope of practice, implementing nursing orders and procedures as specified
  • Educate individuals served on their illness, treatment, and prognosis, reinforcing teaching under the direction of a Registered Nurse
  • Thoroughly and accurately document all aspects of care, communicating pertinent information to incoming staff at shift report
  • Maintain strict confidentiality of individuals served information in accordance with 42 CFR
  • Provide health assessments to individuals served upon admission when possible
  • Prepare and submit nursing assessments and progress notes in accordance with organizational policy
  • Monitor medication logs for accuracy and use EMR/staff logs to convey pertinent information
  • Serve as liaison to area and regional medical facilities and as a resource for individuals served and their families
  • Provide ongoing in-service training and consultation to staff, and participate in continuing education and staff development activities

Requirements

  • High School Diploma or GED required
  • Current Massachusetts Board of Registration license as a Licensed Practical Nurse
  • Minimum one year of nursing experience preferred
  • Two years of sobriety preferred
  • Skilled in applying the nursing process to individuals served and their families
  • Able to collaborate with and appropriately delegate to other caregivers
  • Knowledgeable in utilization/review standards, quality assurance, and CARF standards
  • Able to understand, empathize, and work effectively with an addicted population, including those with co-occurring disorders
  • Working knowledge of the ethics and legal parameters of Substance Use Disorder treatment
  • Excellent verbal and written communication skills
