Long Term Care Nurse

About the role

San Mateo County Health’s San Mateo Medical Center (SMMC) is seeking experienced Long Term Care Nurses to join our on-campus skilled nursing unit.

Responsibilities

• Complete patient assessments and triage, record patient histories, symptoms and observations.
• Establish and implement nursing care plans for each patient through assessment, development, activation and evaluation.
• Participate in patient case conferences and rounds.
• Organize, assign and supervise duties and responsibilities of other staff members as a team leader; orient and teach new staff members of the nursing team and students.
• Perform nursing care following prescribed rules, policies, protocol and procedures.
• Identify problems, assist in data gathering, propose solutions and participate in evaluation of quality assurance issues.
• Work with patient and their families to provide in-hospital and after-hospital follow-up through teaching and referral.
• Participate in educational programs to maintain level of expertise.
• Participate in non-clinical assignments, which may include maintaining records, preparing reports and/or serving on special committees.
• Maintain records and prepare reports.
• Operate a variety of medical machinery and equipment.
• Administer treatment and medications.
• Maintain knowledge of outpatient resources.
• Develop discharge plans for patients.
• Perform related duties as assigned and as required by assigned work setting, as described below.

Requirements

• Possession of a California license as a Registered Nurse or equivalent and a valid Basic Life Support (BLS) certification.
• Knowledge of: Nurse Practice Act, current nursing and related medical theory and skills necessary to administer nursing care within assigned clinical setting, safety and infection control practices, clinical systems, supplies and equipment.
• Ability to: Nurse Practice Act, perform nursing procedures required for assigned clinical setting, comply with clinical procedures and policies, maintain professional nurse-patient relationships, communicate with and educate patients, families, and the public who come from a diverse background, work harmoniously with other personnel, follow safety practices, assist in the instruction of other nursing and paraprofessional personnel.

Qualifications

• At least two years of paid nursing experience in a skilled nursing, long-term care, or comparable complex patient care setting.
• Experience serving as a Charge Nurse, team leader, relief lead, or in another role involving the coordination of nursing staff and patient care.
• Ability to organize and assign work and provide lead direction to nursing and paraprofessional personnel.
• Comfortable responding to changes in patient condition, prioritizing competing clinical needs, and making sound decisions during evening, night, or variable shifts.
• Strong knowledge of skilled nursing and long-term care practices, including medication and treatment administration, care planning, patient assessment, and continuity of care.
• Effective communication with interdisciplinary teams and clear communication with patients, families, providers, and staff.
• Dependable, flexible, and willing to support coverage needs across assigned shifts to help maintain safe and consistent patient care.

Pay

The pay range for this position is $60,000 - $80,000 annually, depending on experience and qualifications.

Schedule

The schedule for this position is typically 0.6 FTE PM/Night Shift.
