Long Term Care Clinical Nurse Manager, RN

Qualifications

• Licensed and currently registered to practice as a R.N. in the state of Minnesota
• Minimum of 3 years of nursing experience in long term care with progressive responsibility
• Working knowledge of state/federal regulations, comprehensive assessments, HER/computer literate, care planning, quality assurance and reimbursement systems
• CPR certified
• Supervisory experience with strong clinical, leadership, interpersonal (written and verbal), and critical thinking skills
• Flexible, dependable, cooperative and willing to be a change agent in a fast paced environment
• Able to delegate tasks, prioritize proactively, solve problems creatively and effectively mentor/coach others
• Strong personal and professional ethics and integrity with the ability to communicate empathetically and effectively with residents, staff, visitors and other health care providers

Responsibilities

• Develops and maintains a comprehensive, individualized plan of care for assigned residents in collaboration with designated staff
• PARTNERS WITH THE INTERDISCIPLINARY TEAM IN EFFORTS TO IMPLEMENT INTERDISCIPLINARY SERVICES IN ASSIGNED NEIGHBORHOOD(S)
• ASSUMES THE RESPONSIBILITY FOR THE IMPLEMENTATION, COMPLETION, AND MAINTENANCE OF RESIDENT ASSESSMENTS, CARE PLANS AND CARE CONFERENCES; DIRECTS AND SUPPORTS OTHER TEAM MEMBERS WITH COMPLIANCE
• MAKES SOUND, LOGICAL AND APPROPRIATE DECISIONS ABOUT RESIDENT ASSESSMENT AND CARE UTILIZING THE NURSING PROCESS
• DIRECTS/MONITORS/SUPERVISES THE PROVISION OF RESIDENT CARE BY LICENSED/CERTIFIED AND OTHER STAFF DURING WORK HOURS; CONSULTS WITH STAFF DURING OFF-HOURS AS NEEDED
• INFORMS PHYSICIAN, RESPONSIBLE PARTY, RESIDENT AND STAFF OF A RESIDENT’S CHANGE IN CONDITION AND TREATMENT PLAN
• ACTIVELY PARTICIPATES IN CLINICAL MEETINGS, CARE CONFERENCES, ON COMMITTEES AND IN OTHER FACILITY ACTIVITIES AS ASSIGNED
