Qualifications
- Licensed and currently registered to practice as a R.N. in the state of Minnesota
- Minimum of 3 years of nursing experience in long term care with progressive responsibility
- Working knowledge of state/federal regulations, comprehensive assessments, HER/computer literate, care planning, quality assurance and reimbursement systems
- CPR certified
- Supervisory experience with strong clinical, leadership, interpersonal (written and verbal), and critical thinking skills
- Flexible, dependable, cooperative and willing to be a change agent in a fast paced environment
- Able to delegate tasks, prioritize proactively, solve problems creatively and effectively mentor/coach others
- Strong personal and professional ethics and integrity with the ability to communicate empathetically and effectively with residents, staff, visitors and other health care providers
Responsibilities
- Develops and maintains a comprehensive, individualized plan of care for assigned residents in collaboration with designated staff
- PARTNERS WITH THE INTERDISCIPLINARY TEAM IN EFFORTS TO IMPLEMENT INTERDISCIPLINARY SERVICES IN ASSIGNED NEIGHBORHOOD(S)
- ASSUMES THE RESPONSIBILITY FOR THE IMPLEMENTATION, COMPLETION, AND MAINTENANCE OF RESIDENT ASSESSMENTS, CARE PLANS AND CARE CONFERENCES; DIRECTS AND SUPPORTS OTHER TEAM MEMBERS WITH COMPLIANCE
- MAKES SOUND, LOGICAL AND APPROPRIATE DECISIONS ABOUT RESIDENT ASSESSMENT AND CARE UTILIZING THE NURSING PROCESS
- DIRECTS/MONITORS/SUPERVISES THE PROVISION OF RESIDENT CARE BY LICENSED/CERTIFIED AND OTHER STAFF DURING WORK HOURS; CONSULTS WITH STAFF DURING OFF-HOURS AS NEEDED
- INFORMS PHYSICIAN, RESPONSIBLE PARTY, RESIDENT AND STAFF OF A RESIDENT’S CHANGE IN CONDITION AND TREATMENT PLAN
- ACTIVELY PARTICIPATES IN CLINICAL MEETINGS, CARE CONFERENCES, ON COMMITTEES AND IN OTHER FACILITY ACTIVITIES AS ASSIGNED
