Works within an 11‑person interdisciplinary care team. The PACE program includes clinic, community, assisted living, SNF, and home visits.

Responsibilities

• 4‑6 patients per shift
• PACE program – clinic, community, assisted living, SNF, and home visits

Requirements

• Geriatric medicine experience required – adult patients 55+
• BLS certification required

Pay

Negotiated better pay deposited weekly.

Schedule

Monday‑Friday, 8 am‑5 pm (8‑hour shifts, guaranteed hours daily). Ongoing assignment – minimum 2 weeks per month, full‑time preferred.
