Local Contract Physical Therapist - $63 per hour

Local contract opportunity in Nashua, NH for a Physical Therapist to provide outpatient rehabilitation services in an in-home setting for the older adult population. Assignment starts 09/07/2026, lasts 13 weeks, with a 40-hour work week and an 8-hour shift from 08:00 AM to 04:30 PM.

About the role

The Physical Therapist will evaluate and treat patients in their private residences or independent living facilities, delivering individualized therapy programs to restore, reinforce, and enhance performance. This role focuses on promoting independence and productivity while addressing comprehensive patient needs through referrals to other disciplines. The position offers a flexible schedule and work environment.

Responsibilities

• Supervise Physical Therapy Assistants, temporary licensees, students, and support personnel in accordance with state licensure requirements and professional standards.
• Adhere to department and patient treatment schedules, including timely clinical and billing documentation and attendance at work-related meetings.
• Attend and contribute to patient care and staffing conferences.
• Make referrals to other disciplines and agencies to address comprehensive patient needs.
• Consult with the rehabilitation team to select appropriate therapeutic regimens based on patient needs and capabilities.
• Adapt programs and plans of care to meet individual patient needs.
• Promote a safe environment and enforce fire and safety hazard elimination.
• Order, clean, maintain, and report equipment needs and repairs.
• Prioritize patient service, ensuring high-quality care in a compassionate atmosphere that recognizes individual needs and rights.
• Screen, examine, and evaluate patients, including history, systems review, and application of tests and measures to establish diagnoses, impairments, and predicted levels of improvement.
• Develop treatment goals and methods in collaboration with patients and caregivers, implement treatment plans, and complete related documentation.
• Provide therapeutic techniques, including:
  • Therapeutic exercise (aerobic capacity conditioning, balance coordination, agility training, body mechanics, postural stabilization, flexibility, gait and locomotion training, neuromotor development, relaxation, strength, power, and endurance training).
  • Functional training.
  • Manual therapy.
  • Airway clearance techniques.
  • Integumentary repair and protection.
  • Use of electrotherapeutic, physical agent, and mechanical modalities.
• Provide health and wellness education, chronic disease management, falls assessment, and interventions to reduce fall risk.
• Perform case management in the home and community.
• Design, fabricate, fit, and train patients in the use of adaptive devices, including orthotics and prosthetics.
• Select activities suited to the patient’s physical and mental capacity, intelligence level, and interest.
• Instruct patients in mobility skills and techniques, evaluate progress, and modify treatment plans as needed.
• Perform routine reexaminations to progress or modify the plan of treatment.
• Incorporate health and wellness programming into treatment plans.
• Perform home and community mobility assessments and provide safety recommendations.
• Perform other related duties as required.

Requirements

• Valid Physical Therapy license in the state(s) of practice or proof of license eligibility.
• One of the following educational qualifications:
  • Master’s degree in Physical Therapy.
  • Master’s degree in a Physical Therapy-related field plus a Bachelor’s degree in Physical Therapy.
  • Bachelor’s degree in Physical Therapy plus 5 years of progressively responsible PT experience.
• Foreign-earned Physical Therapy degrees must be evaluated as equivalent to a U.S. degree in Physical Therapy.
• Valid driver’s license or reliable, independent transportation to and from patients’ residences.

Compliance

• Comply with applicable legal requirements, standards, policies, and procedures, including those within the Compliance and Ethics Program, Standard/Code of Conduct, Federal False Claims Act, and HIPAA.
• Participate in required orientation and training programs.
• Promptly report concerns and suspected incidences of non-compliance to a supervisor, Compliance Liaison, or the Compliance Officer via the Integrity Hotline.
• Cooperate with monitoring, audit functions, and investigations.
• Participate in quality assurance and process improvement activities as requested.

Schedule

• Duration: 13 weeks (14 weeks total length of assignment).
• Hours: 40 hours per week, guaranteed minimum.
• Shift: 8 hours, 08:00 AM – 04:30 PM.
