• Carmel Richmond (Staten Island) – 1 OT
• Ocean Gardens (Queens) – 1 OT
• Outpatient locations: Hillside (Queens) – 1 OT; Jackson Heights (Queens) – 2 OT/OTA; Nicholas Avenue (Manhattan) – 1 OT; 3rd Avenue (Manhattan) – 1 OT; Leonard Street (Manhattan) – 1 OT; Hone Avenue (Bronx) – 2 OT/OTA
• Home Health: Brooklyn, Bronx, Manhattan (Downtown & Uptown), Queens (Astoria, Long Island City, Jamaica), Staten Island – OTs needed throughout

Responsibilities

• Perform comprehensive occupational therapy evaluations
• Develop individualized treatment plans focused on functional independence
• Collaborate with nursing, PT, SLP, and social work teams
