Local contract opportunity for an Occupational Therapist in Farmington Hills, Michigan.

About the role

13-week contract position with a 40-hour work week, 8-hour day shifts. Primary coverage areas include Wayne County, Troy, and Utica, with additional service in Oakland, Livingston, Washtenaw, Genesee, and Macomb counties. Mileage reimbursement is provided between patient visits.

Responsibilities

  • Perform discipline-specific evaluations to determine the need for occupational therapy services.
  • Develop and implement an occupational therapy plan of care.
  • Train patients in activities of daily living (ADLs).
  • Provide caregiver education.
  • Complete accurate documentation.
  • Conduct reassessments of home health patients.
  • Perform admission/comprehensive assessments when no skilled nursing services are ordered and therapy services are included in the referral.

Requirements

  • Graduate of an accredited occupational therapy program.
  • Previous home health experience and familiarity with OASIS documentation, home health regulations, Conditions of Participation, functional assessment, fall prevention, ADL/IADL training, home safety evaluations, adaptive equipment recommendations, and care coordination with other disciplines.
  • HCHB (HomeCare HomeBase EMR) experience.
  • Current CPR/BLS certification.
  • Unrestricted Occupational Therapy license in the state of practice.

Dress Code

Navy blue scrubs or slacks with a navy blue polo-style shirt.

Pay

$52/hr to $58/hr.
