Local Contract Home Health Physical Therapist - $58-60 per hour

About the role

Pride Health is actively hiring a Physical Therapist to join our client's Home Health Program in a growing care team serving patients in Easton, Maryland (covering Caroline, Queen Anne, and Talbot counties). The position is a 13‑week local contract requiring 40 hours per week, Monday–Friday, 8:00 AM–4:30 PM, with a guaranteed weekly hour guarantee and no on‑call or weekend requirements.

Responsibilities

• Perform comprehensive physical therapy evaluations and ongoing reassessments to identify impairments, functional limitations, and safety risks
• Develop, implement, and adjust individualized care plans to improve mobility, strength, balance, endurance, pain management, and functional independence
• Deliver skilled interventions including therapeutic exercise, gait training, transfer training, balance activities, stair training, and home safety education
• Educate patients and caregivers on home exercise programs, fall prevention, assistive device use, and energy conservation techniques
• Complete accurate and timely documentation (evaluations, visit notes, progress reports, and discharge summaries) using EMR systems
• Coordinate care with physicians and interdisciplinary team members, including nurses, occupational therapists, speech therapists, and home health aides
• Communicate changes in patient condition, safety concerns, or barriers to progress promptly to the care team
• Participate in case conferences, care coordination activities, and team collaboration
• Ensure compliance with CMS regulations, infection control standards, and agency policies
• Manage a mobile caseload efficiently while meeting productivity and documentation expectations
• Adapt quickly to new environments, workflows, and patient populations as a travel clinician

Requirements

• Graduate of an accredited Physical Therapy program
• Active Physical Therapist license in the state of assignment
• At least 1 year of home health experience and experience with HCHB
• Willingness to travel within the assigned coverage area
• Current BLS certification
• Valid driver’s license

Pay

$55.00 – $60.00 per hour (W‑2) plus mileage reimbursement (from first patient to last patient). Pay is negotiable and based on experience, expertise, credentialing, and education.

Schedule

Monday – Friday, 8:00 AM – 4:30 PM; 40 hours per week guaranteed; no on‑call or weekend requirement.
