A local contract Home Health Physical Therapist position is available in Ft Mitchell, Kentucky.
Responsibilities
- Provide physical therapy services to patients in their home environment.
- Develop and implement individualized treatment plans based on patient needs.
- Document progress and communicate with referring physicians and other healthcare providers.
Requirements
- Current licensure as a Physical Therapist in the state of Kentucky.
- Experience in home health care setting preferred.
- Valid driver’s license and reliable transportation.
Qualifications
- Master's degree in Physical Therapy from an accredited program.
- ACLS certification.
