Local Contract Home Health Physical Therapist - $43-47 per hour

Host Healthcare is seeking a local contract Home Health Physical Therapist for a local contract job in Paducah, Kentucky.

Responsibilities

• Provide physical therapy services to patients in their homes.
• Develop and implement individualized treatment plans based on patient needs.
• Assess patient progress and modify treatment plans as necessary.
• Document patient progress and treatment sessions accurately.
• Collaborate with other healthcare professionals as needed.

Requirements

• Current licensure as a Physical Therapist in Kentucky.
• Experience in home health setting preferred.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Valid driver's license and reliable transportation.

Qualifications

• Master's degree in Physical Therapy from an accredited program.
• Passing scores on the National Physical Therapy Examination (NPTE).
• Current CPR certification.
