LMSW Program Coordinator for Children Mobile Crisis Team

About the Role

The Program Coordinator supervises the administrative and clinical operations for a Behavioral Health Services (BHS) program, ensuring quality and appropriate utilization of services through an interdisciplinary team approach aligned with VNS Health's strategic goals.

Responsibilities

• Organizes program components, including triage, case assignment, referrals, liaison activities, and staff scheduling to ensure adequate coverage.
• Communicates program policies and procedures to staff and ensures compliance, taking corrective action as needed.
• Oversees maintenance of updated case records through established platforms and ensures compliance with Care Management policies, VNS Health standards, and regulatory requirements.
• Ensures appropriate record-keeping of client information, interventions, and individualized service plans to meet quality standards.
• Maintains volume and productivity to meet program standards, deliverables, and contractual requirements.
• Oversees compliance with quality and performance indicators, tracks client progress, and produces detailed reports.
• Coordinates and collaborates with community-based service providers to ensure continuity of care and promotes cooperative relationships.
• Participates in program audits, ensuring accuracy and clinical competency for client services.
• Ensures program financial targets are consistently met.
• Educates and supervises staff regarding client safety during field visits.
• Plans and maintains 24/7 on-call coverage and performs on-call duties as needed.
• Participates in interdisciplinary team meetings, supervisory meetings, and required training.
• Represents VNS Health on internal and external committees when requested.
• Investigates client complaints, completes incident reports, and addresses client safety and quality issues.
• Identifies intra-team problems and recommends resolutions, including additional training or counseling.
• Supervises and evaluates interns as deemed appropriate by program management.
• Performs supervisory duties, including staff training, interviewing candidates, evaluating performance, and recommending hiring, promotions, or terminations.
• For Mobile Crisis: Participates in after-hours triage, conducts field visits with the Mobile Crisis team, and engages in special projects.

About the Program

The Children’s Mobile Crisis program delivers immediate behavioral health crisis response services for children and adolescents, including psychiatric assessment, stabilization, and de-escalation.

Requirements

• Master's Degree in Social Work, Education, or a related field from an accredited college or university.
• Minimum three years of experience in a community mental health setting.
• Minimum one year of supervisory experience.
• Effective oral, written, and interpersonal communication skills.
• Proficient with personal computer skills, including Microsoft Word and Excel.
• Crisis intervention experience preferred.
• For Children’s Services: Prior experience working with children and adolescents required.

Qualifications

• Valid New York State driver’s license may be required based on program needs.
• For Health Home, CCM, and Wellness: Current license and registration to practice as a Social Worker, Psychologist, Marriage and Family Therapist, Mental Health Counselor, or related license in New York State preferred.
• For Children’s Health Home Program: Child and Adolescent Needs and Strengths New York (CANS) certification. Must complete necessary training to administer the CANS NY assessment in the UAS system within 60 days of start date preferred.

Pay

USD $70,200.00 - USD $87,700.00 per year.
