About the Role
Join Rehab Without Walls, an innovative neurorehabilitation program that delivers care in the patient’s natural home and community environment. We help individuals who have experienced brain injury, spinal cord injury, or stroke/CVA regain functional capabilities and independence through a continuum of care.
Responsibilities
- Perform comprehensive evaluations, assessing the social, emotional, and support needs of patients and their families.
- Set measurable objectives in conjunction with the rehab team.
- Assist physicians and other health team members in understanding significant social and emotional factors related to the patient’s health problems.
- Instruct and counsel patients and families on treating and coping with social and emotional responses connected with illnesses.
- Participate in care coordination activities and act as a resource to other health team members in identifying and resolving patient needs.
- Identify and assist patients and/or families in utilizing appropriate community resources to achieve measurable objectives.
Requirements
- Master’s Degree in Clinical Social Work (LCSW).
- Current Licensed Clinical Social Worker (LCSW) licensure in the state of practice.
- Minimum of one year of experience in a rehabilitation setting serving individuals with traumatic brain injury, spinal cord injury, or acute diagnosis.
- Current BLS (Basic Life Support), First Aid, and CPR certification.
- Ability to lift 50 pounds; may involve lifting up to 100 pounds following safety procedures.
- Duties require fine motor skills, visual acuity, and walking/standing for extended periods.
- Additional physical requirements include pushing/pulling, bending/stooping, reaching, kneeling, and positioning frequently.
