Minimum Required Qualifications

• Oregon LMSW (Masters-level Social Worker) or CSWA (Clinical Social Work Associate) licensure
• Healthcare Provider Level BLS certification
• Use of employee's personal automobile with valid driver's license and auto liability/property damage insurance as required by law
• Must maintain vehicle in good working order
• 1 year experience in specialty

Preferred Qualifications

• Oregon LCSW (Licensed Clinical Social Worker)
• Experience as a social worker in a hospital or medical setting
• Previous hospice experience

Schedule

• Length of assignment: 13 weeks
• Shift: 8:00am–4:30pm, no weekends
• Start date: ASAP
