Essential Job Duties
- Notifies patients of their HIV, HCV and/or STI diagnoses.
- Provide necessary linkage to care services for substance abuse, mental health services, healthcare for persons living with HIV (PLWH), and comprehensive prevention services, as indicated.
- Provide individual and relationship-based HIV/HCV and STI counseling and testing.
- Manage and analyze track, and report data to meet surveillance, evaluation, and reporting objectives of Baptist Hospital and our Grant funders.
- Prepares and submits descriptive and analytical monthly, quarterly, and annual reports, fact sheets and presentations in a timely manner.
- Fulfills internal and external data requests, and performs other related work as required.
- Performs analysis of data, and uses statistics to interpret the significance of data trends.
- Likens the public to resources and information by providing related statistics to the Preventative Screening Dept. community agencies, and the general public.
Required Education And Experience
- A High School Diploma or Equivalent.
- Clinical Chart Review And Data Abstraction Experience.
- Preferred 2 years of clinical healthcare experience or 1 year of experience in social work, case management, or health education/promotion.
