Linkage to Care Specialist - Preventative Screening Services - Days

Essential Job Duties

• Notifies patients of their HIV, HCV and/or STI diagnoses.
• Provide necessary linkage to care services for substance abuse, mental health services, healthcare for persons living with HIV (PLWH), and comprehensive prevention services, as indicated.
• Provide individual and relationship-based HIV/HCV and STI counseling and testing.
• Manage and analyze track, and report data to meet surveillance, evaluation, and reporting objectives of Baptist Hospital and our Grant funders.
• Prepares and submits descriptive and analytical monthly, quarterly, and annual reports, fact sheets and presentations in a timely manner.
• Fulfills internal and external data requests, and performs other related work as required.
• Performs analysis of data, and uses statistics to interpret the significance of data trends.
• Likens the public to resources and information by providing related statistics to the Preventative Screening Dept. community agencies, and the general public.

Required Education And Experience

• A High School Diploma or Equivalent.
• Clinical Chart Review And Data Abstraction Experience.
• Preferred 2 years of clinical healthcare experience or 1 year of experience in social work, case management, or health education/promotion.
