About the role

The LIMHP Bridge Care Therapist at Children's Healthcare provides short-term, solution-focused therapy to pediatric patients and families awaiting ongoing outpatient or psychiatric care. This role bridges the gap between referral and long-term treatment, ensuring timely access, stabilization, and continuity of behavioral health services.

Responsibilities

  • Provide brief, evidence-based therapy (typically 1–8 sessions) focused on stabilization, coping, and connection to ongoing care.
  • Conduct assessments, develop treatment plans, and ensure timely documentation in alignment with clinical and regulatory standards.
  • Ongoing monitoring and updating patient/family safety plans.
  • Collaborate with Behavioral Health Intake, Outpatient Therapy, and Psychiatry teams to promote patient safety, support transitions, and reduce wait times.
  • Coordinate with Intake, Psychiatry, and community partners to ensure warm handoffs and continuity of care.
  • Participate in case consultations, quality improvement, and outcome tracking to strengthen Bridge Care program effectiveness.
  • Support crisis intervention, safety planning, and provider consultation as needed.

Requirements

This position requires a Master's Degree in Counseling, Social Work, Psychology, or a related behavioral health field. Three years of experience as a licensed mental health practitioner in a child or adolescent environment are required, with three years of providing therapy to children, adolescents, and families preferred. One year of experience in healthcare and/or behavioral health settings is also preferred.

  • Licensed Independent Mental Health Practitioner upon hire.

  • Knowledge of child/adolescent development, trauma, and family systems (high proficiency).

  • Effective organizational and time management skills (medium proficiency).
