Lifespan Outpatient Clinician

Provides clinical and case management services for children and adults across the lifespan in an integrated health care setting that provides primary care, behavioral health, and dental services. Fosters positive working relationships with co-workers, clients, and external key stakeholders.

$10,000 sign-on bonus: half paid at start, the other half paid at 1 year of service. Eligible for up to $250,000 in loan repayment through National Health Service Corps (NHSC). Federally Qualified Health Center (FQHC) in Hartford, CT.

Pay

Salary:

• $60,000.00 for Associate Licensure (LMSW, LPCA, LMFTA)
• $73,500.00 for Full Licensure (LCSW, LPC, LMFT)

Requirements

• Master’s degree in Marriage and Family Therapy, Counseling, Social Work, or related license-eligible field, or a doctoral degree in Psychology
• Must be licensed or license-eligible (LPCA, LMFTA, LPC, LMFT, or Psychologist)
• If you are an MSW, a LMSW (permit or license) or LCSW will be required to begin employment

Responsibilities

• Works as part of a multidisciplinary team to meet the behavioral, primary, and dental health needs of clients, including:
  • Providing behavioral health consultation to primary care providers
  • Assisting clients in meeting medical treatment compliance
  • Referring clients to other Wheeler services or community resources as needed
• Conducts program-specific screening and evaluation, formulates diagnoses in accordance with the DSM
• Develops and documents clear, individualized, and measurable treatment plans in collaboration with the client/family, inclusive of behavioral health, primary care, and dental goals
• Demonstrates knowledge of psychopathology and primary psychotherapy modalities (individual, group, and family)
• Correctly triages clinical emergencies, including suicide and danger to others
• Uses best practices and evidence-based psychotherapy models where appropriate
• Facilitates topic-oriented, focused, time-limited group therapy
• Demonstrates knowledge of psychotropic medications
• Completes clinical documentation in a timely manner
• Facilitates group therapy (outpatient or intensive outpatient) and sees patients across the lifespan as necessary
• Demonstrates knowledge of internal and external levels of care, referral processes, and resources
• Makes clinically appropriate referrals for identified service needs in a timely manner
• Communicates effectively with clients, families, and internal/external providers
• Attends required meetings and multidisciplinary treatment team planning sessions
• Meets direct service productivity goals as defined by the program
• Adheres to program/department/agency goals, practice standards, and data collection/reporting protocols
• Documents all client-related information in compliance with agency policy
• Attends and prepares for scheduled supervision; seeks additional supervision or consultation as needed
• Collaborates with community providers to promote Wheeler Clinic’s reputation as a quality service provider
• Provides culturally competent care, respecting clients' cultural traditions, values, and beliefs
• Reports child abuse/neglect as mandated; adheres to confidentiality guidelines
• Ensures the program meets all necessary licensing, regulatory, and accreditation standards
• Continues to develop knowledge and understanding of the populations served, using culturally appropriate approaches
