Position Overview
The Life Story Program Director is responsible for overseeing the development, implementation and management of specialized dementia care programming within the facility. This role ensures that all memory care services are person-centered, engaging, meeting the interests, physical, mental, spiritual and psychosocial well-being of residents and align with best practices and regulatory standards.
Key Responsibilities
- Design, implement, and evaluate structured daily memory care programming tailored to residents’ cognitive abilities and interests.
- Ensure activities align with person-centered care principles and dementia best practices.
- Upholds and ensures Resident Rights, including privacy, dignity and quality of life.
- Adheres to infection control standards, HIPPA and COVID precautions.
- Participates in ongoing in-servicing and dementia training along with staff.
- Maintains healthy communication with residents’ families and care teams.
- Educates staff and residents’ families about dementia while working with families to maintain residents’ support systems.
- Collaborates closely with staff to monitor resident engagement in activity programs and interactions with peers.
- PARTNERS WITH COMMUNITY MEMBERS.
- ORGANIZES THE ACTIVITY CALENDAR TO MEET RESIDENTS’ NEEDS WHILE ADHERING TO STATE AND FEDERAL REGULATIONS, POST THE CALENDAR AND DISTRIBUT TO RESIDENTS.
- PROVIDES, OR ENSURES A DESIGNATED STAFF MEMBER PROVIDS, ONE TO ONE ACTIVITIES FOR RESIDENTS WHO REQUIRE SUCH ACTIVITIES.
- COORDINATES ACTIVITY PROGRAMS.
- Completes resident and/or family interviews, assessments including MDSs, CAAs, individualized care plans, and updates documentation, such as progress notes, as required.
- ADHERES TO A PROGRAM THAT MEETS THE MEDICALLY RELATED SOCIAL AND EMOTIONAL NEEDS OF ALL RESIDENTS.
- WILL PROVIDE PERSONAL ORATORY DEMENTIA SPECIFIC TRAINING TO EVERY EMPLOYEE WHO ROUTINELY WORKS THE SECURED DEMENTIA UNIT.
Qualifications
- Hold a degree from an educational institution in a health care, mental health, or social service profession or be a licensed health facility administrator.
- Minimum of one (1) year’s work experience with dementia or Alzheimer's residents, or both, within the past five (5) years.
