About the role

Supports the delivery of high-quality, person-centered, outcome-based adult day services to individuals with intellectual and developmental disabilities. Directly assists individuals with planning and selecting activities and life skills learning experiences, and achieving personal goals that foster self-awareness, growth, and independence. Provides emotional support, supervision, and personal care while guiding and coaching entry-level Life Skills Coaches and direct care associates.

Responsibilities

  • Assists individuals with assessments, planning, and evaluation processes.
  • Supports individuals in preparing for and attending meetings with their support team, advocating and facilitating changes to the service plan as needed.
  • Assists in developing and implementing an Individual Service Plan (ISP) consistent with the individual’s personal goals.
  • Maintains required documentation and data, including ISPs, quarterly and annual evaluations, and case notes.
  • Collaborates with service consultants (OTs, LVNs, Behavioral Consultants, etc.) on enhanced service delivery and completes related documentation.
  • Supports an individual's behavior plan when indicated.
  • Assists individuals with selecting person-centered activities and life skills learning experiences aligned with their desires, ISP, and IPP objectives.
  • Works with other service members to plan and deliver activities consistent with individual and service objectives.
  • Supervises individuals at the service and provides personal care support, including nutrition, personal hygiene, and appearance.
  • Supports individuals in communicating their needs and choices, educating them on appropriate tools to help them find their voice.
  • Provides guidance to Life Skills Coach 1 and direct care associates as necessary.
  • Safely and effectively uses technology, alternative communication, and adaptive equipment related to individuals and the service.
  • Trains individuals in the use of assistive technology as directed.
  • Follows procedures, standards, guidelines, and best practices to ensure health, safety, compliance, and quality of services.
  • Attends in-service trainings and meetings as required.
  • Performs other duties as assigned.

Requirements

  • High School diploma.
  • Valid CPR and First Aid Certifications (provided by employer for Adult Day Services).
  • Non-violent crisis management/intervention certifications (e.g., CPI, QBS) required for services supporting individuals with challenging behaviors.
  • 4-6 years of related experience preferred; one year of experience working with individuals with challenging behavior required for behavioral services.
  • Ability to lift up to 50 pounds repetitively.
  • Ability to walk, stoop, stand, and kneel for extended periods.
  • Ability to push wheelchairs on a variety of terrain.
  • Ability to tolerate an outdoor work environment, including various weather conditions.
  • Ability to move and rearrange furniture (e.g., tables and chairs for activities).
  • Ability to travel locally with individuals to participate in service-related activities.
  • Must obtain and maintain criminal record/fingerprint clearance from the Department of Justice and FBI.
  • Must provide proof of required vaccinations or positive titer showing immunity.
  • Must pass post-offer medical examination, tuberculosis test, and drug testing.

  • Intermediate proficiency with Microsoft Office applications (Outlook, Excel, Word, PowerPoint).
  • Ability to assist with personal care, including transferring individuals with no weight-bearing skills.
  • Ability to resolve conflicts and remain calm in stressful environments.

Pay

Starting pay rate: $22.00 per hour.

Schedule

Full-time, Monday–Friday, 8:00 AM to 2:00 PM.
