Responsibilities
- Providing Supports, Interventions & Services
- Supporting people with typical daily activities
- Supporting effective communication between the person supported and others
- Assessing needs to identify any conditions or behavior patterns that require medical interventions and supports
- Providing crisis prevention and intervention services
Maintaining Health & Safety of the Individuals
- Completing tasks directly related to maintaining and ensuring health
- Following individualized care plans to best support the person’s overall health and safety
- Communicating all identified needs with the care team and family
- Lifting and transferring to assist people with mobility, facilitating motion exercises and physical therapy
Engaging Individuals in Socially Valued Roles
- Advocating for maximum independence and community inclusion for people supported
- Empowering people to make their own choices
- Offering education, training, and self-development for people in accordance with the Individual Support Plan (ISP)
- Building and maintaining relationships with important people in the person's life (family, friends, supporters)
Documentation & Administrative Requirements
- Maintaining the content, integrity, and security of all documentation
- Demonstrating proficiency in the use of technology in documentation
- Accurately and thoroughly completes and submits timely, all required documentation

Qualifications
- High School Diploma or GED
- Nursing Assistant Certification (CNA), preferred
- Must be 18 years of age
- Reliable means of transportation
- Valid Driver’s License, with excellent driving record
- The ability to lift and transfer 50 pounds without restrictions
- Excellent communication and writing skills
- Act 33, 34 and FBI Clearances
- Flexible schedule - willing to work varying days and hours based on the person/family’s needs

Pay
Minimum Compensation: USD $18.50/Yr.
Maximum Compensation: USD $20.00/Yr.
