Responsibilities

• Assist the Activities team in creating a positive, engaging, and supportive environment for adults receiving long-term care and mental health services
• Plan and lead recreational, social, educational, and therapeutic activities that encourage participation, recovery, independence, and meaningful connections
• Work collaboratively with the Activities team to support each resident's individual goals and recovery journey; communicate any concerns to the Activity Director
• Monitor participation, document progress, and help evaluate the effectiveness of programs
• Bring fresh ideas to create engaging events, seasonal celebrations, and meaningful daily activities
• Build positive relationships with residents while promoting social interaction, confidence, and meaningful engagement
• Create a warm, welcoming, and inclusive environment where every resident feels respected, valued, and encouraged
• Maintain a safe, organized, and positive activity environment
• Perform other duties or responsibilities as assigned

Requirements

• High school diploma or equivalent required
• Experience working in a long-term care or post-acute care setting is preferred, but not required
• Basic computer knowledge required
• Ability to read, write, speak, and understand English
• Must be able to push, pull, move, and/or lift a minimum of twenty-five pounds
• Ability to stand for one-hour intervals for up to 75% of the workday
• Must successfully pass a pre-employment physical examination

This center is managed by Transitional Care Management, providing comprehensive management services to several inpatient healthcare centers throughout Northern IL and the Chicagoland area, including Long-term Care Skilled Nursing (SNFs), Short-term Care Rehabilitation, Intermediate Care (ICFs), and Behavioral Health Service (SMHRFs) centers.
