About the Role

This position involves evaluating, writing, monitoring, and coordinating behavior management plans with the clinical team to support direct care client services. The Licensed Independent Clinical Social Worker (LICSW) or Licensed Clinical Mental Health Counselor (LCMHC) will work directly with patients, primarily children, in a school-based setting.

Responsibilities

  • Provide direct treatment to patients as needed.
  • Ensure service quality and participate in care coordination to facilitate communication between clinical associates, patients, and referral sources.
  • Participate in performance improvement activities.
  • Investigate case situations and present relevant patient and family social background information to physicians and the health team for diagnosis and treatment.
  • Interpret psychiatric treatment to patients' families to reduce resistance and promote acceptance of care and continuation of treatment.
  • Refer patients or families to community resources as needed.
  • Help patients understand, accept, and follow medical recommendations.
  • Support, consult, and train staff on substance abuse and mental health issues.
  • Document progress, data, and service plans in the Electronic Medical Record (EMR) using objective, measurable methods.
  • Consult with the clinical team and participate in clinical team meetings.
  • Write psychosocial reports for inclusion in a patient's Individual Plan of Service and ensure they are added to the Electronic Medical Record.
  • Demonstrate knowledge and skills necessary to provide age-appropriate care and understand growth and development principles across the lifespan.
  • Perform other assigned duties, which may include varied work hours and placement within ACHS.
  • Maintain current knowledge of clinical practices.

Requirements

  • Licensed Independent Clinical Social Worker (LICSW) or Licensed Clinical Mental Health Counselor (LCMHC).
  • At least three years of professional clinical experience.
  • Current NH license or license eligible.
  • Valid driver’s license.

Skills

  • Strong oral communication skills.
  • Basic experience and knowledge of computer systems and programs.
  • Organizational skills, self-motivation, and ability to perform multiple office tasks.

Work Environment

This position operates in a professional clinical environment with constant interaction with others and routine multitasking. Standard office equipment such as computers, phones, photocopiers, scanners, and fax machines will be used regularly.

Physical Demands

  • Regularly required to see, hear, and speak.
  • Frequent walking, standing, moving, and manipulating equipment.
  • Frequent use of hands for handling, reaching, and keyboard motion.
  • Manual dexterity required for safe performance of patient care tasks.
  • Required to sit, stoop, kneel, or crouch.
  • Specific vision abilities include close vision, distance vision, ability to adjust focus, and peripheral vision.
  • Daily lifting of 20-25 pounds; occasional lifting up to 50 pounds may be required depending on care provided.
