Clinical Social Worker, LC or LICSW

About the Role

The Social Worker is responsible for performing social work assessments and interventions for inpatients, outpatients, and emergency department patients. This role is dedicated to patient and family-centered care, valuing personal self-determination, managing multiple priorities, and engaging in creative, compassionate, and ethical problem-solving. As a member of a multidisciplinary health team, the Social Worker performs comprehensive assessments and develops treatment plans integrating medical, social, and resource issues impacting individual patients, families, and at-risk populations.

This position requires direct patient care and involves collaborating with all healthcare team members to develop and implement treatment plans supporting patient-centered care.

Responsibilities

• Provides psychosocial assessments of patients and families with social, emotional, interpersonal, and/or environmental issues.
• Formulates biopsychosocial assessment, disposition, and treatment plans.
• Collaborates with other professionals to evaluate patients' medical or physical condition and assess client needs as part of the interdisciplinary team.
• Monitors, evaluates, and records client progress according to measurable goals described in the treatment and care plan.
• Demonstrates proficient clinical judgment and application of professional values and ethics.
• Integrates prior clinical experience into current practice.
• Formulates assessments of risks, crisis intervention, psychosocial assessments, treatment plans, and documents them in a timely manner.
• Demonstrates sound clinical judgment and innovation in advocating for clients while maintaining professional boundaries.
• Provides a range of evidence-based interventions.
• Advocates, coordinates, and continuously communicates treatment plans with patients, family, and the interdisciplinary team.
• Incorporates social sensitivity.
• Utilizes supervision and consultation regularly and appropriately when needed.
• Actively seeks assistance with complex cases and situations in a timely manner.
• Engages in quality improvement projects and uses data to measure progress.
• Facilitates skill-based groups.
• Negotiates and works effectively within a complex organization.
• Communicates findings effectively with a broader audience.

Requirements

• Master's Degree in Social Work (required).
• Licensed Clinical Social Worker (LCSW - State License) required.
• Independent Clinical Social Worker (LICSW - State License) preferred.
• 2-3 years of clinical experience in a medical setting required.

This is an onsite position located at 2014 Washington Street.

Scheduled weekly hours: 40.

Pay

Pay range: $67,537.60 - $98,238.40 per year (Annual, Grade 7).

Base pay is determined by skills, relevant experience, education, certifications, and other essential factors. The provided range is based on minimum job qualifications and does not encompass all elements of total compensation.
