Responsibilities
- Administering medications and treatments, monitoring responses, and reporting changes promptly
- Aiding with mobility, transfers, and activities of daily living (ADLs) to support therapy goals and promote independence
- Monitoring vital signs and neurological status to detect changes and respond quickly
- Documenting progress and functional ability to guide care planning and team coordination
- Educating and supporting patients and families about recovery milestones, safety, and self-care techniques
- Working closely with nurses, therapists, and physicians to deliver personalized, high-quality care
Qualifications
- Required Licensure/Certifications: Current LVN Licensure that allows the practice in the state of Texas or Compact state, Basic Cardiac Life Support certification
- Required Education/Experience: Licensed Vocational Nursing or Associates Degree
- PREFERRED EDUCATION, LICENSURE, CERTIFICATION, EXPERIENCE, SKILLS, KNOWLEDGE AND ABILITIES: Reads and writes Basic English with the basic skills necessary to transfer physician’s orders, Ability to listen, empathize and respond to patient, family, internal and external partners needs, Rehab Nursing experience preferred, Effective communication skills, both orally and written, Computer skills needed, Ability to manage a team and work with others within a team to ensure quality patient care, Strong problem solving skills, Uses the FIM instrument to assess patient function level of independence throughout the rehabilitation program, Bilingual in English/Spanish preferred.

The position is available on a PRN (Per Request Notified) schedule.
Pay
The pay rate for this position is competitive and commensurate with experience.
