Responsibilities

  • Provide direct nursing care including medication administration, phlebotomy, and documenting all nursing-related assessments in the EMR.
  • Conduct daily physical and mental status assessments and document them accordingly.
  • Complete health questionnaires and accurately document in the medical record.
  • Administer medication, observe for side effects and client response, and document observations in the medical record.
  • Create and maintain weight and vital signs in EMR, check daily for accuracy, and recheck any unusual documentation.
  • Participate in client observations to promote abstinence of behaviors and safety.
  • Coordinate with pharmacy for medication deliveries and questions regarding medications.
  • Fax additional information as needed for payments according to HIPAA protocols and facility policy.
  • Monitor the nutritional and dietary needs of clients and report non-compliance with the plan of care.
  • Organize, coordinate, and follow up on all consultations with physicians/medical personnel for tests and provide appropriate diagnoses for laboratory, radiology, and other diagnostic testing.
  • Complete all Alsana Nursing Assessments, Care-plans, S.O.A.P Notes, Admission Note, Discharge Notes, NOC Shift note, and accurately document in the medical record.
  • Ensure all LVN Notes are co-signed by the Nurse Manager and/or RN.
  • Administer first aid according to facility policy, including all appropriate follow-up documentation.
  • Obtain insurance updates to get precertification for medications and other coverage.
  • Establish and maintain good communication with clients, families, and staff.
  • Participate in “hand-off” communication from shift to shift.
  • Perform all other duties as assigned.

Requirements

  • IV certification within 30 days of start date.
  • Valid license as a Vocational Nurse.
  • 1+ years hospital-based clinical experience preferred.
  • Current CPR Certification, preferably through the American Heart Association.
  • 1+ years clinical experience in a mental health or residential facility is preferred.

Schedule

PRN NOC 7pm - 7am
