Licensed Vocational Nurse (LPN/LVN)

About the role

This full-time position expects 30 productivity points completed per week, with pay calculated per point. Clinicians have the opportunity to earn additional pay by performing more than 30 productivity points weekly. The breakdown is $44.33 per point, with an approximate annual pay range of $75,000.00 - $100,000.00.

Responsibilities

• Provide comprehensive care in the patient's home, including skilled documentation through Homecare Homebase.
• Serve as the patient's advocate and maintain ongoing communication with physicians, referral sources, caregivers, and facility management to coordinate care.
• Administer medications and treatments as prescribed by a physician.
• Provide direct patient care, including infusion therapy, wound care, teaching, and disease management.
• Participate in regular case conferences and in-services.
• Service approximately 6 patients per day, Monday through Friday.

Requirements

• Current Texas LPN license.
• Three years of clinical nursing experience.
• Current Texas driver's license and CPR certification.
• Experience with healthcare software and competency in carrying out skilled procedures within the Nurse Practice Act.
• Strong organization skills, time management, and ability to multitask while meeting timelines.
• Professional appearance and effective communication skills.
• Comfortable treating wounds and performing infusion therapy.
