Licensed Vocational Nurse - Community Living Center

About the Role

We are seeking a Licensed Vocational Nurse (LVN) to join our Geriatrics and Extended Care team at the Community Living Center, working the night shift. This role focuses on providing advanced clinical care for Veterans with complex, behavioral, or chronic conditions, emphasizing resident-centered care, safety, and interdisciplinary collaboration.

Responsibilities

• Provide advanced clinical care for Veterans with complex, behavioral, or chronic conditions.
• Perform clinical tasks such as wound care, catheter management, enteral feeding, and respiratory support within scope.
• Participate in care planning, prioritize resident needs, and document in CPRS and BCMA systems.
• Administer medications safely, educate residents and caregivers, and recognize adverse reactions.
• Deliver trauma-informed, dignity-centered, and resident-focused care promoting independence and engagement.
• Collaborate with interdisciplinary team members, communicate clinical findings, and participate in team meetings.
• Ensure safety, infection control, and participate in quality improvement initiatives.
• Serve as a role model and assist with staff orientation, demonstrating leadership and teamwork.

Qualifications & Skills

• Current, unrestricted LVN license.
• Experience in nursing care, medication administration, and emergency response.
• Ability to work independently and as part of a team.
• Proficiency in documentation and communication.
• Physical ability to perform required tasks.
