About the Role

We are searching for a Licensed Vocational Nurse (LVN) to assess, observe, evaluate, and document patient interventions. The LVN will provide teaching, counseling, support, and quality patient care while working as part of a team. Familiarity with community health resources, social services, and welfare programs is required.

Responsibilities

  • Take patient history, vital signs, symptoms, and conditions, and document them in the medical record.
  • Check patient’s height and weight.
  • Administer screening tests, evaluate and record results.
  • Administer patient immunizations following clinical protocols and provider’s instructions.
  • Administer medications and treatments ordered by the provider.
  • Maintain lab duties, including daily quality checks, specimen processing, result documentation, and provider notification of abnormal results.
  • Perform daily cleaning and disinfection of lab and equipment in accordance with infection control guidelines.
  • Submit QC and patient testing logs monthly.
  • Check treatment rooms for properly functioning equipment and stock supplies.
  • Provide individual or group health education to patients, including distributing educational materials.
  • Review chart documentation for completeness according to established indicators.
  • Travel to other CentroMed sites as needed.
  • Abide by infection control, hazardous materials, and safety policies.
  • Demonstrate competence with patients aged 0 and older.
  • Participate in the agency’s Quality Improvement Program.
  • Perform other duties as required.

Requirements

  • Graduation from an accredited school of Vocational Nursing.
  • Current Texas LVN license.
  • CPR Certification.
  • Clinic or hospital experience preferred.
  • Bilingual/Bicultural (Spanish) required.

Physical Requirements

  • Ability to perform the essential functions of the position.
  • Must meet attendance requirements.
  • Must not pose a threat to the health or safety of others in the workplace.
  • Physical ability to lift up to 20 pounds unassisted.
  • Ability to deliver excellent customer service, both externally and internally.
