Licensed Social Worker - Social Medicine

About the Role

Provides psychosocial assessments, coordinates, and implements age-appropriate social medicine services in emergency and CDU settings, supporting patients and families.

Responsibilities

• Participates as part of the care team, addressing high-need populations such as those with new diagnoses, terminal illnesses, social issues, or homelessness.
• Completes psychosocial assessments, develops care plans, and ensures documentation per standards and regulations.
• Supports patients and support systems with coping strategies and connects them to community resources post-discharge.
• Provides social medicine consultations on abuse, neglect, advance care planning, and legal decisions.
• Contributes to discharge planning, ensuring appropriate community placements and education on risks and benefits.
• Offers resource referral, advocacy, and mediation services, and participates in departmental development, quality improvement, and staff training activities.

Qualifications

• Minimum one year healthcare experience.
• Master’s degree in Social Work.
• Licensed Clinical Social Worker or Licensed Social Worker.
• Basic Life Support certification.

Skills & Knowledge

• Experience working with interdisciplinary teams and understanding medical terminology and psychosocial impacts of illnesses.
• Strong interpersonal, communication, and customer service skills.
• Knowledge of community resources, family dynamics, and care planning processes.
