Responsibilities
- Affords assistance to the patient and family with health-related financial, social, and emotional concerns.
- Provides social work services in line with the plan of care.
- Ensures timely communication of changes in the patient's clinical status to other healthcare team members and care center office staff.
- Educates the patient and family members, as well as other team members, on medical social work interventions and treatment plans.
- Performs care planning, transfer, and discharge functions.
- Identifies risks or threats to the patient's health in the patient's environment and implements appropriate interventions to reduce these risks.
- Communicates identified risks/threats to the physician and other members of the healthcare team to coordinate care effectively.
- Performs other duties as assigned.
- Holds a master’s or doctoral degree from a school of social work, accredited by the Council on Social Work Education.
- Has one year of social work experience in a healthcare setting.
- Mets state and federal requirements for this position.
- Currently holds a valid CPR certification.
