Licensed Social Worker – Mount Carmel Medical Group

About the Role
Franklinton Medical Group is establishing a walk-in/same-day clinic to meet community needs as we transition away from providing ED services in the area. The Social Worker provides care coordination, connects patients with community resources, and addresses behavioral health and social needs. This position will help ensure patients receive timely support and continuity of care in an accessible outpatient setting.
Requirements
- Bachelor degree in Social Work
- Current LSW license to practice as a social worker in the State of Ohio
- Minimum 1 year experience in discharge planning in an acute care setting or an ambulatory setting
- Effective communication skills
- Strong organization skills, self-directed, flexible, able to adapt to rapidly changing regulatory requirements
- Excellent clinical assessment and analytical skills
- Computer competency
- Effective interpersonal skills; ability to work across departmental boundaries, facilitate problem resolution, and maintain a professional demeanor in difficult situations
- Knowledge of the functional operations of utilization management, discharge planning, care coordination, and transitions of care
- Understanding of pre-acute and post-acute venues of care and post-acute community resources
- Ability to work independently and exercise sound judgement in interactions with physicians, payers, colleagues, patients, and their families
- Maintains a professional demeanor
