Licensed Social Worker
Responsibilities
Provides patient assessment, counseling, and discharge planning.
Acts as a liaison between the patient/family/hospital and community agencies/resources.
Demonstrates expertise in social work, working as a member of an inter-disciplinary team comprised of a physician, nurses, and case management.
Applies specialized knowledge of human development and behavior and social, economic, and cultural systems in directly assisting patients and families to improve or restore their capacity for social functioning. This includes counseling and use of psychosocial interventions and social psychotherapy.
Occasionally plans educational offerings and provides in-services as requested.
Requirements
Bachelor’s Degree in Social Work required.
Current, valid, and active license to practice as a Social Worker in the state of employment required.
Master of Social Work preferred.
Two (2) years of experience as a Social Worker in a healthcare setting preferred.
Qualifications

Knowledge of regulatory standards and compliance requirements.
Strong organizational, prioritizing and analytical skills.
Ability to make independent decisions when circumstances warrant.
Skills
Working knowledge of computer and software applications used in job functions.
