Licensed Social Worker

Essentials Duties and Responsibilities

• Provides direct clinical treatment to residents as represented in the caseload through: diagnostic evaluations and screenings, crisis/emergency assessment and intervention, treatment planning and assessment.
• Participates in interdisciplinary and planning meetings.
• Consults with doctors, individuals, and other social services agencies, insurance providers and hospital networks regarding clients, client services and case management.
• Maintain client records and administrative records according to Valley View's policy.
• Attends administrative staff meetings as required.
• Attends in-service education, clinical supervision and professionals meetings as required.
• Perform related duties as assigned.

• Can develop and implement treatment plans which are behaviorally oriented, measurable, and appropriate for the identified level of care.
• Clinical expertise specific to the target population with an ability to engage clients effectively and work with other professionals and organizations.

Qualifications

The ideal candidate will have:

• A Master’s degree in Social Work from an accredited college or university.
• A valid Texas Social Work License.
• At least 2 years of experience in a healthcare setting.
• Experience in discharge planning and social services.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.

Skills

• Strong clinical skills and knowledge of social work principles.
• Proficiency in Microsoft Office Suite.
• Excellent organizational and time management skills.
• Ability to work under pressure and manage multiple tasks simultaneously.
