Responsibilities
- Conduct psychosocial assessments for new admissions and ongoing residents.
- Develop and implement individualized care plans in coordination with the interdisciplinary team.
- Provide counseling and emotional support to residents and families.
- Aid in discharge planning and coordination of community resources and services.
- Serve as a liaison between residents, families, and facility staff.
- Document all assessments, interventions, and care plans accurately and timely.
- Advocate for residents’ rights and ensure compliance with state and federal regulations.
Qualifications
- Bachelor’s Degree in Social Work (BSW) required; Master’s Degree (MSW) preferred.
- Current NJ Social Work license (LSW or LCSW) required.
- Experience In Long-term Care Or Sub-acute Rehabilitation Setting Preferred.
- Strong interpersonal, communication, and organizational skills.
- Caregiver-centered approach to care.
