Licensed Social Worker

About the role

We are seeking a full-time Licensed Social Worker to help launch a new PACE Center in Plainfield, NJ. The position offers a salary range of $70,000 - $80,000, a full complement of benefits, and paid time off.

Responsibilities

• Perform in-person initial assessments for enrollment of potential Lutheran Senior Healthcare participants to gather a complete psycho-social history, including cognitive status, social supports, family dynamics, mental health, and substance dependency.
• Coordinate with the interdisciplinary team to develop a comprehensive care plan for each participant.
• Provide psychosocial assessment and individual, family, and group counseling.
• Maintain accurate records and prepare clear and concise reports, correspondence, and other written materials.
• Communicate effectively with all types of groups, both verbally and in writing.
• Work in an interdisciplinary team setting, collaborating with physicians, nurses, therapists, and other healthcare professionals.

Requirements

• Member of the Academy of Certified Social Workers (ACSW).
• Current driver’s license and proof of auto insurance.
• Licensed with the New Jersey Board of Social Work.
• MUST be English/Spanish bilingual.
• Experience with frail/chronic elderly people.
• Two years experience working on an interdisciplinary team in a hospital, nursing home, or community-based setting is preferable.
• One year working with the frail or elderly.
• Medical clearance for communicable diseases and up-to-date immunizations.

Qualifications

• Education: Bachelor’s degree in Social Work or related field.
• Experience: Two years experience working on an interdisciplinary team in a hospital, nursing home, or community-based setting is preferable. One year working with the frail or elderly.
