Job Expectations

Aid in planning, organizing, implementing, evaluating, and directing the operations of the Social Service Department. Provide medically related social work services to help residents achieve or maintain optimal physical, mental, and psychosocial health.

Job Responsibilities

• Assist with resident and family interventions
• Provide psychosocial services
• Offer vision, hearing, and dental services
• Conduct assessments and care planning
• Develop discharge plans

Requirements

• At least 2 years of experience in a Skilled Nursing Facility or Hospital
• Four-year degree in Social Work
• Licensed Social Worker in the State of Texas

Education / Qualifications

• Four-year degree
• Licensed Social Worker in the State of Texas

Knowledge / Experience

• At least 2 years experience in a Skilled Nursing Facility or Hospital
