Looking for a meaningful part-time role where you can truly make a difference? Ledgewood Rehabilitation & Skilled Nursing Center in Beverly is seeking a compassionate and motivated Social Worker to join our team.
Responsibilities
- Compiles social history data.
- Participates in assessments, development and evolution of resident's care plan.
- Responsible for planning and leadership interdisciplinary care planning weekly meetings.
- Documents timely progress notes according to state and federal regulations.
- Captes discharge planning for residents.
- Able to interpret and implement established social service policies, procedures and regulations.
- Captions services of community agencies for continued delivery of care upon discharge.
Requirements
- Must possess a B.A in Social Work, LSW or equivalent degree AND a current social work license.
- Masters in Social Work (MSW), and LCSW or LICSW higher preferred.
- 1+ years skilled nursing facility experience preferred.
