Schedule: Part-Time (16 hours/week)

Requirements

• Bachelor’s degree in Social Work (B.A.), LSW, or equivalent degree
• Current social work license
• Master’s in Social Work (MSW) and LCSW or LICSW preferred
• 1+ years of experience in a skilled nursing facility preferred

Responsibilities

• Compile social history data
• Participate in assessments, development, and evolution of resident’s care plan
• Plan and lead weekly interdisciplinary care planning meetings
• Document timely progress notes according to state and federal regulations
• Coordinate discharge planning for residents
• Interpret and implement established social service policies, procedures, and regulations
• Coordinate services of community agencies for continued delivery of care upon discharge
