Licensed Social Worker

Responsibilities

• Conduct psychosocial assessments and develop individualized service plans.
• Provide counseling and case management to clients across behavioral health, housing, and family services.
• Coordinate community resources to support client needs.
• Collaborate with an interdisciplinary team to ensure comprehensive care.
• Maintain accurate documentation of client interactions and service plans.
• Advocate for clients, promoting dignity, inclusion, and social justice in a supportive environment.

Qualifications & Skills

• Valid license as a Social Worker.
• Experience in behavioral health, housing, or family services preferred.
• Strong interpersonal, communication, and advocacy skills.
• Ability to work collaboratively within a team.
