About the role

As a Behavioral Health Therapist at MyCHN (Community Health Network), you will provide comprehensive behavioral health services for patients of all ages while collaborating with an integrated care team to improve overall health outcomes.

Responsibilities

  • Conduct behavioral health assessments and develop individualized care plans
  • Gather patient histories through interviews with patients and families
  • Provide individual, couples, family, group, and crisis counseling services
  • Support wellness management and coordinate referrals for specialized care when needed
  • Collaborate with providers, psychiatrists, and interdisciplinary care teams
  • Educate patients on behavioral health conditions and health-promoting activities
  • Ensure timely and accurate documentation of all patient interactions
  • Manage follow-up care and ensure timely communication with patients
  • Support integrated care coordination across behavioral health, primary care, and community resources
  • Perform additional duties as assigned within scope

Requirements

  • Active and unrestricted Texas license as an LCSW or LPC
  • Ability to practice independently
  • Eligibility for Medicare/Medicaid provider participation
  • At least one year of clinical experience in area of practice
  • Strong communication and interpersonal skills
  • Ability to work effectively with diverse populations
  • Ability to handle sensitive situations and complex patient needs
  • Strong collaboration and care coordination skills
  • Commitment to patient-centered care and positive patient outcomes

Qualifications

  • Bilingual in Spanish (preferred)
  • Knowledge of public health principles and integrated care models (preferred)
  • Experience working with substance use disorders (SUD) (preferred)
  • Five years of clinical experience (preferred)
